3/6/2024 EnchantaVaughn Approved QMHPA greeted Ms. Broadnax upon arrival to the home. Upon arrival to the home, Ms. Broadnax appeared to be in a relaxed mood evident by her facial expressions. QMHPA inquired about her day. Ms. Broadnax verbalized to QMHPA that her day was going. Ms. Broadnax did not present any symptoms of SI or HI at the time of the session being conducted. QMHPA engaged Ms. Broadnax in a one on one discussion to discuss her current thoughts, feelings and emotions. QMHPA encouraged Ms. Broadnax to discuss with QMHPA how she truly feels certain situations that she is challenged with on a daily basis. QMHPA engaged Ms. Broadnax in a discussion to determine if she had taken her medication to ensure consistent mood stability to assist her with some of those anger outbursts that she is currently experiencing. After communicating with Ms. Broadnax on her presenting issues and finding solutions. QMHPA transported Ms. Broadnax out into the community to allow her the opportunity to increase her ADL’s. QMHPA allowed Ms. Broadnax this opportunity by allowing her to budget her monthly benefits to provide groceries for family. QMHPA thanked Ms. Broadnax for acting accordingly in the community and being respectful to her peers and presenting positive interactions. Ms. Broadnax was receptive to QMHPAs’ intervention. Ms. Broadnax indicated to QMHPA that she is just tired of people taking advantage of her. Ms. Broadnax verbalized to QMHPA that she thinks that her family just didn’t care about her and her family as was thinking about self. Ms. Broadnax indicated to QMHPA that she is going to begin to think about self as well. Ms. Broadnax verbalized to QMHPA that there a lot of things that she is going to have to do when it comes to her family members and the way that she is getting treated. Ms. Broadnax verbalized to QMHPA that she has been adhering to her medication management because that is the only thing that appears to keep her calm. She indicated to QMHPA that she also has to ensure that she is adhering to her medication management in order for her to continue to receive her disability. Ms. Broadnax thanked QMHPA for escorting her out into the community to purchase some household items. Ms. Broadnax also indicated to QMHPA that she is going to attempt to control her anger on a daily basis. QMHPA will continue to offer therapeutic techniques to assist Ms. Broadnax in processing symptoms and roots of depression
